Health Goals:
To improve the
health of our children and reduce ethnic inequalities in
health outcomes for certain targeted preventable
conditions and reduce the rates of hospitalisation in
primary school aged children for these health conditions
(e.g. rheumatic fever and skin infections) to national
average pākeha rates or below as measured using hospital
discharge data.
Health Aims:
1. To increase the
number of assessments and treatment
in primary school aged children for targeted
conditions (e.g. Group A
beta-haemolytic streptococcal sore throats and
skin infections ) that potentially
result in hospitalisation if left untreated as
well conditions traditionally managed by Public Health
nurses.
2.
To increase the
health knowledge of all members of the school community.
3.
To increase health
literacy of all members of the school community.
Objectives:
1. To
increase access to primary care for primary
school aged children for certain

Population Served:

Mayfield Primary
School, Otara, Auckland
Service Objectives:
Registered Nurse (RN) led, primary school-based health
service enables RNs to address a number of important
health issues. The RNs work closely with the school
communities to maximise the opportunity for assessment
and treatment, and to deliver broad health
promotion/education messages as well as working with
families in order to improve the general health and
wellbeing of both children and their whānau.
